For Healthcare Providers
Your patients don’t need more information. They need help turning recommendations into real life.
You already know the problem.
You explain the diagnosis. You review the labs. You recommend nutrition changes, strength training, better sleep, stress management, alcohol reduction, weight loss, glucose monitoring, or medication adherence.
The patient nods. They understand. They may even be motivated.
Then they go back to work, travel, family obligations, decision fatigue, old habits, and a life that does not rearrange itself around the care plan.
That is where coaching helps.
I help patients implement the plan
I help patients translate clinical recommendations into practical routines they can actually follow.
My work sits in the space between the medical plan and the patient’s real week.
I help patients clarify what matters most, identify the barriers getting in the way, and build sustainable systems around nutrition, movement, strength training, sleep, stress, recovery, and follow-through.
This is not about giving patients more generic advice. It is about helping them act on the advice they already have.
Experience in clinical care
I spent 18 months as a member of the care team in a concierge primary care practice, where I worked with patients to translate physician recommendations into practical behavior-change plans around nutrition, movement, strength, sleep, stress, cardiometabolic health, and follow-through.
That experience shaped the way I coach. I understand the gap between a sound medical recommendation and a patient’s ability to follow through in real life. I also understand the importance of clear communication, scope of practice, documentation, and continuity between the coaching relationship and the clinical care team.
I help your patients achieve their health & wellness goals.
Health coaching can be especially useful for patients working on:
Cardiometabolic risk reduction
Weight loss or weight maintenance
GLP-1 support, including protein, strength training, and muscle preservation
Nutrition consistency, including protein, fiber, meal structure, and reduced added sugar
Strength training and exercise adherence
Sleep routines and stress regulation
Alcohol reduction or other behavior-pattern changes
Follow-through after lab review, body composition testing, or physician recommendations
Sustainable routines for busy professionals, caregivers, and people with real-life constraints
What coaching can support
PATIENT OUTCOMES IMPROVE WITH HEALTH COACHING.
Most patients do not fail because they lack information.
They struggle because implementation is complicated.
The plan may be clinically sound, but the patient still has to answer practical questions:
What do I eat for lunch when I’m in meetings all day?
How do I start strength training if I’m deconditioned, injured, or overwhelmed?
What is the first change that will move the needle without blowing up my life?
How do I keep going after the initial burst of motivation fades?
What do I do when travel, stress, caregiving, or work disrupts the plan?
Coaching helps patients answer those questions and stay engaged long enough to test, adjust, and refine what works.
Just a few of my happy, healthy clients…

I’m a 56-year-old single mother with a very demanding job. When I met Sarah, I was scared and overweight. Sarah and I developed a plan that worked for me. With Sarah’s support and humor (she is very witty) I stayed the course. I lost 24 lbs and 19 inches in 4 months with Sarah. A year later I am down 30 lbs and 24 inches. I feel fantastic and I am back to me. I could not have gotten here without Sarah. - S.A.
-
Before Sarah, I felt powerless and desperate regarding my weight. Sarah and I easily formed a very manageable plan that allowed me to implement change immediately. Despite holidays, vacations, work stress and lots of family activities, 7 months later I’m 22 pounds down and I feel so much better about my overall health. Jeans fit that I wouldn’t have dared to try on last year. To always have someone so clearly in my corner in this journey was transformational. I feel hopeful and in control of this process. –N.P.

Sarah taught me strategies to deal with the chaos that is real life. She supported me unconditionally throughout the process of losing nearly 40 pounds. I feel better in my own skin; I went down three sizes; I have more energy; and my cholesterol numbers are much better. Moreover, I learned how to cook healthy meals, shop strategically at the supermarket, and eat balanced snacks. I came away with a toolbox of healthy habits that’ll help me going forward. –L.H.

I LOVE how supportive and insightful Sarah is - she asked me questions about how certain food choices made me feel and taught me how to really make a connection between what I ate and my mood, energy level, etc. She’s sensitive and flexible - I appreciate her vast knowledge about so many health and wellness topics, but it's her relatable approach that really stands out for me! -M.H.

Before working with Sarah, I was over well over my ideal weight, had hip and joint pain, and was feeling uninspired to get back to focusing on my health. After 3 months, I’m not in pain, I’ve gained strength, lost weight, and just feel really good about myself. -S.C.

I love Sarah’s individualized approach and the ability to tweak my plan as needed. After working with Sarah, I feel fitter, stronger, have more energy, and am sleeping better. I have takeaways for life: increased fiber intake and an exercise program that doesn’t require hours at the gym but that’s effective, efficient, and feels good. Sarah helps to take the guesswork out of health and wellness, and she gets results. -L.C.
How do my services work?
I work with clients individually or in groups. I work in-person, via Zoom, or over the phone, to coach them through healthy lifestyle changes, so they can overcome any barriers standing in their way
My coaching is practical, evidence-informed, and behaviorally grounded.
I help patients:
Identify the highest-leverage next step
Turn broad recommendations into specific weekly actions
Build routines around their actual schedule and constraints
Reduce all-or-nothing thinking
Troubleshoot barriers quickly
Track what matters without overcomplicating the process
Create accountability that supports autonomy rather than dependence
The goal is not perfection. The goal is durable progress.
A partner in implementation
I am not a replacement for medical care, diagnosis, or treatment.
I support patients in implementing the care plan, staying engaged, and building the habits and systems that make clinical recommendations more likely to hold.
When coaching is well integrated, patients feel less alone, clinicians are not left trying to provide behavior-change support in a 20-minute visit, and the care plan has a better chance of becoming lived behavior.
Who is a good candidate for referral?
I work well with patients who understand what they’ve been advised to do, but need help making it happen in real life.
Good candidates may be working on cardiometabolic health, weight management, strength, nutrition, sleep, stress, alcohol reduction, GLP-1 support, or follow-through after a new diagnosis, lab result, or physician visit.
The best fit is someone ready to engage, willing to experiment, and open to practical accountability between medical visits.
What’s the next step?
If you have a patient who could use support implementing health recommendations, please reach out or use the referral form below.
I use a HIPAA-compliant platform for referrals, client communication, documentation, and updates.
After the referral, I’ll connect with the patient to clarify their goals, current barriers, and readiness for coaching. If we decide to work together, coaching will focus on translating the care plan into practical routines around nutrition, movement, sleep, stress, recovery, and follow-through.
With the patient’s consent, I can share brief updates so you know where coaching is focused and how the patient is progressing.
